Registration at Dancing Moose
DMMS is open from 6:30 am to 6:00 pm and offers part-time as well as full-time programs for children ages 18 months through 6 years old.

Registration
In order to save a spot in your desired program please fill out the Enrollment Form and pay the $65 enrollment fee.  If you would like us to send you the enrollment forms via email, please send a request to info@mydancingmoose.com.  You can fax the completed forms to (801) 966-1135.  In order to finish the enrollment process, the following steps must be taken.

• Fill out the Enrollment Form (front and back)
• Pay $65 Enrollment Fee
• Fill out the Child Health Assessment (a doctor’s signature is not required for this form)
• Provide a copy of immunizations, or ask your child’s Doctor to fax immunization to Dancing Moose
